Knowing your health risks is a key factor to improving your overall health. There are two important actions you can take to get a good sense of your health risks–at no cost to you:
- Visit your primary care provider (PCP) for your annual checkup and
- Complete a health risk assessment (HRA) online through your state health plan.
What is an HRA, and how do I take one?
An HRA is an easy-to-answer, short set of questions about your health, diet, physical activity, sleep habits and medical history that helps you understand your current health and any conditions you might be at risk for. After you finish answering the questions in the online HRA, you’ll get a personal action plan to help you address any health risks your HRA has identified, to help you maintain or improve your well-being.
First, if you haven’t already done so, make an appointment with your PCP and go for your yearly physical exam. Remember, no matter which plan you’re in, you shouldn’t have any out-of-pocket costs when you visit an in-network provider for your preventive exam.
Your PCP can give you a copy of your most recent lab and health screening numbers, such as your blood pressure and cholesterol levels. These health numbers will be helpful to have when you complete your online HRA.
Where can I find an online HRA?
All of the health plans offered through the Texas Employees Group Benefits Program (GBP) provide tools that allow you to access an online HRA.
- HealthSelect of Texas® and Consumer Directed HealthSelectSM plans: If you already have a Blue Access for MembersSM account, log in. If you do not have an account yet, click Register Now and use your medical ID card to create an account. Once you’re logged in, click on Well onTarget® under the Quick Links on the left to access the health assessment.
- Scott and White Health Plan: Log in to your account to take an online wellness assessment and get a health improvement action plan. If you don’t have an account yet, click Log In and then Sign Up Now. Use your medical ID card to create an account and find the online wellness assessment.
- Community First Health Plans: Log in to your account to take an online health assessment. You’ll then get a personalized plan for making healthy lifestyle changes.
Use an app for that! With the BCBSTX Mobile App, you can get to the health assessment at your fingertips, wherever you are. To get the app, text BCBSTX to 33633 to get a link to download the app, or download the app yourself from Google Play or the Apple App store. Log in to your Blue Access for Members account and click Well onTarget to get to the HRA and complete it on your mobile device.

Take the online health risk assessment offered by your health plan to learn your health risks, so you can start taking steps toward a healthier life. Even small fitness and nutrition improvements–like taking a short walk during the day and eating an extra serving of vegetables–can make a big difference to your overall health.
Frequently Asked Questions
1. Who else sees my answers to the HRA?
Your health information is private and secure. Your employer will not see your specific answers to any of the HRA questions. Only aggregate data, which cannot be traced back to you individually, is made available to the health plan and the Employees Retirement System of Texas®. The aggregate health data is information that is collected from various HRAs and compiled into a summary for the health plan and ERS. The aggregate data is simply a measuring tool to get a general health profile of the state workforce.
Your individual HRA results are yours to help you understand your own health and what you can do to improve it. With that in mind, you might want to talk about your HRA results with your PCP. But that’s your decision to make.
2. How long does it take to fill out an HRA?
For HealthSelect participants, the assessment should take 10 minutes or less. For Community First Health Plans and Scott and White participants, the assessment takes 15 to 20 minutes to complete. That’s a pretty small time investment to learn important information about your health!
3. What if I don't know the answers to all of the questions on the HRA?
No problem. You can skip questions you don't have the answers to. But, the more questions you can answer, the more information you will get out of your assessment. See next question and answer for details.
4. How do I get the most out of my HRA?
Most of the questions are multiple choice and simple to answer, but there are a few that need information from a doctor’s visit, lab results, a scale, and a tape measure. Try to have the following information ready when you log in to do your HRA:
- weight (without clothing),
- waist circumference,
- height (without shoes),
- total cholesterol, both your HDL (“good”) cholesterol,
- and LDL (“bad”) cholesterol,
- blood sugar (glucose) level
- blood pressure (systolic and diastolic),
- A1C and
- resting pulse.
5. As a HealthSelect member, can I earn Blue Points for completing the HRA?
Yes, you earn 2,500 Blue Points every time you complete your assessment – as often as every six months. Redeem your Blue Points in the online shopping mall available in Well onTarget. Points can be redeemed in small increments or all at once, up to 17,325 points per year.
6. If I’d like to check my progress, how can I view my HRA in the future?
If you are a HealthSelect participant, you can view past assessments by logging into your Blue Access for Members account on the HealthSelect homepage. Once you are logged into your account, select the Health Assessment tab on the left to take you to the Well onTarget homepage. From the Menu button in the top left of the screen, navigate to the resources page to view past reports.
Scott and White participants can view their most recent assessment by logging in to their SWHP account.
7. I opted out of a GBP health plan. Can I still complete an HRA?
Yes, you may complete the online health assessment on the Scott and White website for non-members. If any of your family members aren’t enrolled in a GBP plan, the HRA on the Scott and White website is also available to them.
