Extra support for those who need it

Do you have questions about specific health concerns or conditions? A Personal Health Assistant can help by connecting you with one of our Texas-based care management clinicians. Whether you have an upcoming surgery and have questions, recently had surgery and need follow up support, or if you were recently diagnosed or are managing a chronic or serious condition, a clinician can support you and your family. Clinicians listen to your concerns and take your health history, lifestyle and behaviors into account when offering guidance.
