Will you be changing your address soon? If you are moving into or out of the service area for your health provider, you can change to a provider available in your area.

Remember to:

• Update your address in your online account.
• Designate a new primary care physician (if applicable).
• Provide new banking information for direct deposit (if necessary).
  • Active employees will contact their HR department or Benefits Coordinator.
  • Retirees will contact ERS
