Some health plans cover an annual vision exam and/or offer discounts on glasses or contacts. Check your health plan’s Master Benefits Plan Document or call your health plan for details. - To make the most of your benefits, get your eye exam and glasses or contact lenses at a provider in the Superior National network, a broad and diverse network of independent ophthalmologists and optometrists including 46 of the Top 50 optical retail chains across the country.
- If you use an out-of-network provider, you will pay more.
- Vision benefits and discounts cannot be combined with any other coverage.
For example: You can use the vision discounts that your health plan offers, or you can use the discounts available through State of Texas Vision, but they cannot be combined.
The services are available every day, all day or night, for emergency assistance and transport when traveling 100 or more miles away from home.
